Cardiosvacular agent; Antihypertensive/ACE inhibitor
Mechanism of Action
Angiotensin-converting enzyme (ACE) catalyzes the conversion of Angiotensin 1 to angiotensin II, which is a potent vasoconstrictive substance. As such, the inhibition of ACE by this drug decreases levels of angiotensin II, which leads to a drop in blood pressure from inhibited vasoconstrictive stimulation. It is also known to lower the pulmonary capillary wedge pressure (PCWP)
Indication
Used in management of mild to moderate hypertension. this drug can be used in combination therapy for CHF or in acute myocardial infarction because the drugs effects deviser both preload and after load to the heart
Contraindications/ Precautions
Pregnancy; history of angioedema related to previous treatment with an ACE inhibitor; pre-existing hypotension; cariogenic shock’ know hypersensitivity to ACE inhibitors
Adverse effects
Hypotension or postural hypotension. The patient may also experience reflexive tachycardia with heart palpitations. Patient may display angioedema, nausea. committing, alterations in kidney function and hyperkalemia

Onset within 1 hour, peak 4-6 hrs, duration at least 24hrs

Special considerations
Use cautiously with patients with renal impairment or renal artery stenosis, on diuretic therapy, or undergoing dialysis
Monitor blood pressure and cardiac rhythm closely
Serial lab values should be monitored closely so that potassium levels can be watched for hyperkalemia
