What is the probable organism responsible for pertussis (whooping cough)?
Bordetella pertussis.
What are the first-line antibiotics for treating pertussis in adults and children?
For Adults: Erythromycin (1-2 grams per day). For Children: Clarithromycin (15 mg/kg/day, divided BID) or Azithromycin (10 mg/kg on the first day, then 5 mg/kg/day x 4 days).
When should chemoprophylaxis be provided for pertussis contacts?
All household contacts, regardless of age and immunization status, should be offered chemoprophylaxis. It is essential in infants < 1 year old or patients in the 3rd trimester of pregnancy residing with or having been in close contact with the index case.
What is the duration of infectiousness in pertussis cases?
The patient is generally considered infectious until 3 weeks after the onset of cough (without therapy) or until 5 days after starting therapy.
What is the preferred treatment for infants with pertussis?
Macrolides, especially azithromycin, are the preferred treatment for infants with pertussis. For children 2 months of age and older, an alternative to macrolides is TMP/SMX.
