Hip Fractures Flashcards
Usual treatment for hip fracture?
Surgery > conservative
Arterial supply of femoral head?
Circumflex femoral arteries- branches of profunda femoris
Possible risks of intracapsular hip fractures?
- Arterial supply of femoral head can be disrupted
- Risk of AVN
- Risk of non-union
Treatment of intracapsular fractures?
Hemi-arthroplasty
Total hip replacement
What is hemi-arthroplasty?
Replacing femoral head alone
When is hemi-arthroplasty preferred?
For restricted mobility & cognitively impaired
What is total hip replacement?
Replacing acetabulum & femoral head
Risks and outcomes of THR?
Higher risk of dislocation
Better functional outcome
What is anther term for extracapsular hip fracture?
Intertrochanteric hip fracture
Prognosis of extracapsular hip fracture?
Shouldn’t cause AVN
High union rate
Treatment for extracapsular hip fracture?
Internal fixation
Compression/dynamic hip screw
What is another term for extracapsular proximal femoral fracture?
Subtrochanteric
Aetiology of Subtrochanteric proximal femoral fractures?
Elderly patient with osteoporosis: fall onto side
Prognosis of extracapsular proximal femoral fracture?
- Subtrochanteric bone has poor blood supply & considerable bending stress
- Fractures take a while to heal
- Non-union occurs fairly frequently
Treatment of Subtrochanteric proximal femoral fractures?
- Strong indirect fixation: intramedullary nail
- Thomas splint: pre-operative pain relief and stabilization of fracture
