Pathology of cubital tunnel syndrome?
Compression of ulnar nerve
-Behind the medial epicondyle (funny bone)
Cause of cubital tunnel syndrome?
- Tight band of fascia forming the roof of tunnel (Osborne’s fascia)
- Tightness at the intermuscular septum as the nerve passes between the 2 heads at the origin of flexor carpi ulnaris
What is the tight band of fascia forming the roof of cubital tunnel syndrome called?
Osborne’s fascia
Signs and symptoms of cubital tunnel syndrome?
- Paraesthesiae in ulnar 1.5 fingers
- Tinel’s test over cubital tunnel = +ve
- Weakness of ulnar nerve innervated muscles
Which muscles are innervated by ulnar nerve ?
1st dorsal interosseous (abduction index finger) Adductor policis (Froment's test)
What does the 1st dorsal interosseous do?
Abduct index finger
Investigation for Cubital Tunnel Syndrome?
Nerve conduction studies confirm the diagnosis
Treatment for cubital tunnel syndrome?
May need surgical release of any tight structures
What is a positive Froment’s test?
Due to weakness of adductor pollicis, patient will compensate action by flexing flexor pollicis longus of thumb to maintain grip strength
