Recurrent headache with symptom free intervals
MIGRAINE
at least 3 of the ff (FRUANT)
family history relief ff sleep unilateral location associated aura abdominal pain nausea and vomiting throbbing in character
Indications for cranial or MRI
abnormal neurologic signs
behavioral changes, recent school failure, fall off in linear growth rate
headache awakens the child during sleep
migraine and seizure occur in the same episode
focal neurologic signs
cluster headache esp < 5y/o
Treatment of Migraine
avoid stimuli - stress, fatigue, anxiety
ACETAMINOPHEN or IBUPROFEN
ANTIEMETICS (Metoclopramide (IV)
Treatment of Status Migrainosus (persistent headache > 3 days)
Prochlorperazine IV
Prophylactic Therapy
more than 2-4 severe episodes monthly
unable to attend school regularly
PROPRANOLOL 10-20 mg TID > 7 days
FLUNARIZINE - 5 mg at bedtime
Indications for prophylactic therapy in children with migraine
> 2-4 episodes
unable to attend school regularly
