Acutely Ill Child Flashcards
The MC reason for a sick child visit
Fever
The first response to unwitnessed unresponsive infant
activate emergency response system
Features of high quality CPR
rate 100/min
compression depth of 1/3 anterior posterior diameter of chest
allow complete chest recoil after each compression
minimize interruptions in chest compressions
avoid excessive ventilation
Factors attributing in definition of clinically significant bradycardia
heart rate systemic hypoperfusion (pallor, altered mental status, hypotension, acidosis)
The MC pre-arrest rhythms in young children
bradyarrhythmia
Atropine is used in treatment of bradyarrhythmia that is associated with
increased vagal tone (increased ICP) or primary AV block
Shockable rhythms
ventricular fibrillation (VF) pulseless ventricular tachycardia (VT)
The perfusion pressure of the brain (cerebral perfusion) equals to
mean blood pressure - ICP
The development of INCREASED ICP with IMPENDING HERNIATION may be heralded by
new onset or worsening headache
depressed level of consciousness
vital signs changes - HPN, bradycardia, irregular respirations
signs of 6th (lateral rectus palsy) or 3rd CN compression(anicosoria)
Important to diagnose brain death status
apnea
(-) brainstem reflex
(-) motor response
(-) confounding factors
Prior to extubation and in order to reduce airway narrowing after extubation it is advised to use
Dexamethasone
Best prognosticators of long term CNS outcome after prolonged CPR
neuro exam
progression during the first 72 hrs
