Neurological Assessment
- Note results from last exam
- Ck medical diagnosis and health history
- If client is alert, explain that they will be asked some question
Ask pt to: -state name
- state where they are
- identify year, month, date, and day of the week
ALWAYS ASSESS VITAL SIGNS
Documentation
- Document assessment techniques performed and specific finding
- Note: cognitive responses of the pts

sensation and motor responses
Reflex testing data - Document any pt stmts of pain, muscle weakness, or joint abnormality
- Record inspection findings, including turgor, color, temp, pulses, and capillary refill
Older adult considerations
Slowed reaction time
Short term memory (recalling events)
Decreased musculoskeletal function (loss of muscle strength)
May take longer to perform certain actions (completing activities for testing coordination)
Reflexes
Assess the integrity or sensory and motor pathways of the reflex arc (neural pathway that controls reflexes) and specific spinal cord segments
