Sense of Vision
Test vision for acuity and visual field.
CN I
Olfactory
Sensory
Sense of smell
Test each nostril for smell reception with various agents and interpretations.
Have pt. smell something with eyes closed.
CN III
Oculomotor
Motor
Pupil constriction and raise eyelids
Test pupillary reaction to light and ability to open and close eyelids.
CN IV
Trochlear
Motor
Downward inward eye movement
Test for downward and inward movement of the eye.
CN V
Trigeminal
Motor jaw movements – chewing and mastication. Ask patient to open and clench jaws while you palate the jaw muscles.
Sensory sensation on the face and neck
Test face and neck for pain sensations, light touch, and temperature.
Use cotton or pin to see if pt can feel it
CN VI
Abducens
Motor
Lateral movement of the eyes
Test movement of the eyes in all directions.
CN VII
Facial
Motor. Muscle of the face. Ask the patient to raise eyebrows, smile, show teeth, and puff out cheeks.
Sensory
Sense of taste on the anterior 2/3 of tongue.
Taste 3 thing. Can they tell the difference
CN VIII
Acoustic Sensory Sense of hearing. Test hearing ability. Do the whisper test see if they can hear you. Use a tuning fork. Hearing and balance
CN IX
Glossopharyngeal
Motor. Pharyngeal movement and swallowing. Ask pt. to say “ah” and have pt. to yawn to observe upward movement of the soft palate; elicit gag response; note ability to swallow.
Sensory. Sense of taste on the posterior 1/3 of tongue. Test for taste using various agents.

Vagus Motor/Sensory Swallowing and speaking Ask pt. to swallow and speak; note any hoarseness. Pharynx Movement of vocal cords
CN XI
Accessory Motor/Sensory Movement of shoulder muscles. Ask pt. to shrug shoulders against your resistance. Head resistance
CN XII
Hypoglossal
Motor
Tongue movement and strength of the tongue
Ask pt. to protrude tongue; ask pt. to push tongue against cheek.
