DDx ground glass infiltrates
Interstitial pna, PCP pna, Pulmonary edema (including ARDs or cardiogenic edema), Hypersensitivity pneumonitis (e.g., methotrexate causes this), Pulmonary hemorrhage (think drug use, e.g., cocaine)
Positive PPD if immunosuppressed pt
> 5 mm induration
Normal A-a gradient
5-15 mmHg
Causes of high A-a gradient
Difficulty getting oxygen from alveoli to arteries: PE, Pulmonary edema (incl. ARDs), Right-to-Left vascular shunt (bypassing pulmonary circulation), High inspired oxygen count
Signs of peritonsilar abscess
Uvula deviation away from abscess; Difficulty opening mouth, Muffled hot potato voice
Empiric tx for PNA: 2 mo. old
Macrolides (azithromycin or erythromycin)
Empiric tx for PNA: 2 yr old
Amoxicillin (oral) or Ampicillin (IV)
Tx atypical PNA
Macrolides (azithromycin or erythromycin)
