GBS
Causes of dislocation of lens of eye
Marfan’s syndrome (upwards); Homocysteinuria (downward); Alport (can’t see, can’t pee, can’t hear); Traumatic
Weber test: conductive v. sensorineural
heard best in affected ear - conductive; heard best in unaffected ear - sensorineural
BPPV dx & tx
Dix-Hallpike maneuver; Epley maneuver
Cherry red spot on macular
Tay-Sachs, Niemann-Pick, Central retinal artery occlusion
Tx macular degeneration
Atrophic - Vitamin supplementation - beta-carotene, C, E, copper, zinc; Exudative - intravitreal ranizumab (VEGF) or laser photocoagulation
Tx retinal detachment
Stop progression of retinal detachment w/ cryotherapy or laser photocoagulation; Reattach retina, which usually helps recover some of vision
Risk factors for cataracts
Trauma, especially caustic substances, Diabetes, Corticosteroid, Older, Sunlight exposure, Tobacco, Low education, Alcohol
Orbital v. Periorbital cellulitis
Orbital - limitation of eye movement, propotosis, pain on eye movement, decreased/double vision
Tx acute angle glaucoma
Eye drops (e.g., beta blockers, cholinergic agoinsts, alpha agonists, prostaglandin analogs) and Acetazolamide; If refractory and dx confirmed, IV mannitol; Laser peripheral iridotomy
Next step chemical burn to eye
Copious water irrigation, immediately
Easiest way to distinguish Hordoleum v. Chalazion
Eyelid margin v. In eyelid itself
Lesion to CN III
Fixed and dilated pupil, down and out eye
