3.2 Nutrition- breastfeeding part 2 Flashcards
What test can be done to detect inverted nipples
pinch test
Type of nipples—everted, flat, or inverted. It is not always possible to detect an inverted nipple by observation only.
The “pinch test” may be needed to identify nipples that invert with tactile stimulation to the areola.
To do the pinch test, place the thumb and forefinger on opposite sides of the areola approximately 1 to 1.5 inches back from the nipple-areolar junction.
Gently compress as though bringing the two fingers together, causing the nipple to become more everted or inverted.
This assessment should be conducted prenatally on every patient
When teaching about breastfeeding how should the baby be positioned
face on at nipple height
so no head tilting or turning is needed
How should the baby be lying with mom to breastfeed
belly to belly with mom
Are clicking and popping sounds normal when breastfeeding
no, good latch results in quiet feedings
Is a glug or air blowing out babys nose normal when breastfeeding
yes- glug is audible swallowing
air blowing out nose as breathing
Once baby is positioned to breastfeed how can mom stimulate the baby to open its mouth
draw nipple from infants nose to chin to stimulate mouth opening
lips should flange (open placement)
If a baby is squished into the moms breast when feeding what can be done
bring bum closer to moms tummy
shoudl be straight line from babys ear to shoulders to hips
Which breastfeeding position is less likely to stimulate the startle (Moro) reflex and support innate feeding reflexes
physiologic laid-back position
In the laid-back position, the mother reclines in a position that supports her back and neck and brings her baby to her chest and allows the baby to rest its cheek against her breast.
The baby is able to use the rooting and sucking reflexes to find the nipple, and the mother can support her breast to help guide the baby.
When the baby has latched, gravity helps to maintain a deeper latch and the mother is able to relax and enjoy the breastfeeding session
What should the mother do when the feeding session is over to prevent pain when the nipple is removed from the mouth
insert her finger between babys lip and breast to break suction before moving baby from breast
When is the ideal first feeding of a newborn
30-60 mins after birth
This first feeding should take place in the delivery area with encouragement by all in attendance. It will not delay, to any significant extent, any procedures required, such as weighing and measuring the infant, instilling ointment or drops in the infant’s eyes, and giving a vitamin K injection.
These procedures can be done while the baby remains on the mother’s abdomen.
The mother and infant should remain together as much as possible, with rooming-in preferable.
How long should the infant be enouraged to feed for in the first session
10-15 mins for each breast to practice latching
infant behaviour is more important than time on clock
How can you stimulate a newborn to wake and suckle
rub back, foot, cheek, or chin
What decreases the chance of cracked nipples
a proper latch
During the first 24 hours how often and long should the infant be feeding
8-12 times in 24 hours (every 2-3 hours) for 20-45 minutes per feeding
frequent suckling stimulates breastilk production
What are two red flags for infants breastfeeding in the first 4-6 weeks
- sleeping for more than 4-5 hours at a time
- falling to sleep on the breast after 5 minutes
This infant must be actively wakened and stimulated for feeding.
