What is the optimal time for scheduling a patient who is currently taking levodopa?
One hour after administering the medication
Levodopa - dopamine replacement agent used to relieve symptoms of Parkinson’s Disease
What is a side effect of cholinergic agents that may impact PT treatment?
Parasympathetic effects - bradycardia, bronchoconstriction
Cholinergic agents - mimic or increase acetylcholine in the body
Increase in Ach -> increased parasympathetic activity
Describe the clinical presentation of myasthenia gravis
Severe muscle weakness
NO loss in sensory or reflexes
Proximal to distal muscle weakness
Describe the indications for antispasticity agents vs muscle relaxant agents
Antispasticity agents - typically used for spasticity or hypertonicity caused by CNS disorders (SCI, CVA, MS, etc.)
Muscle Relaxant Agents - typically used for muscle spasms secondary to MSK or peripheral nerve injuries
Describe considerations for PT treatment in individuals w/ myasthenia gravis
Avoid strenuous exercise
Avoid muscle fatigue
Avoid excessive heat or cold - can exacerbate symptoms
Treatment should be based on patient’s current symptoms, strength, and fatigue level
Monitor respiratory function for signs of weakening of the muscles of respiration
Observe for signs of a myasthenia “crisis” - respiratory difficulty, swallowing issues, labored talking or chewing
Initiate strengthening for patients w/ mild to moderate symptoms by using moderate to maximal isometrics
