- Diverticular hemorrhage
2. Angiodysplasia
Dx for Acute colonic diverticulitis:
CT
Sx of acute diverticulitis:
Pain
Diarrhea
Fever
Abd tenderness
**hemorrhage is rare
Surgical indication in diverticulitis:
perforation
Tx for mild diverticulitis:
oral hydration
liquid diet
abx
Age for acute diverticular hemorrhage
over 50
Tx for acute diverticular bleed:
Resus
- reverse anticoag
- transfuse if necessary
- rule out upper GI bleed
Observe – 80% are self-limited
Tortuous, dilated submucosal capillaries lacking smooth muscle
Angioectasia (Angiodysplasia)
Most common cause of hematochezia in patients over 65:
Angioectasia
Tx for angiodysplasia
Colonoscopy w/ablation
Cause of non-occlusive ischemic colitis:
hypoperfusion
**also drugs, heart disease, exercise
Cause of occlusive ischemic colitis:
embolic
a fib, thrombosis, hypercoag
**Severe, ACUTE, abd pain followed by self-limited hematochezia:
Ischemic colitis
Most common site for ischemic colitis:
Splenic flexure
Management of ischemic colitis:
correct underlying cause (ie. reperfuse tissue)
