angina pectoris heavy tight, squeezing indigestion brought on by physical or emotional exertion Radiates: neck, jaw, arms relieved with nitro
GERD chest pain
clenched fist upper sternum with vertical motion
pericarditis pain
pleuritic in nature sharp/stabbing recent cold? flu-like/virus? worsen with deep breath happens with position change?
pulmonary emboli
pt reports
- sharp chest pain
- SOB or presyncopy w/ hypoTN and peripheral cyanosis

family hx of heart dis. under 60 DM HTN dyslipidemia smoking male obesity
past medical hx
angina pectoris or MI valvular dis irregular rhythm stroke/TIA peripheral vascular disease HTN cholesterol issues thyroid
past surgical hx
bypass surgery stents valve repair pacemakers/defibrillators assist device: IABP, LVAD
medications
beta-blockers calcium channel blockers anticoag/platelets nitro birth control ASA
family hx
long QT marfan DM heart disease dyslipidemia HTN obesity congenital disease
social hx
smoking/tobacco ETOH: how many drinks, what type illicit drugs diet exercise living situation occupation ADL's
infant/children exam
tiring during feeding or playtime breathing changes cyanosis growth/wt gain favored knee-chest position mothers health during preg. headaches nosebleeds joint pain unexplained fevers physical and cognitive development
preg. exam
hx of cardiac disease or surgery
postpartum cardiomyopathy
dizzy/fainting with standing
indictions of heart disease during preg.
dyspnea progressive orthopnea PND HTN syncope with exertion chest pain with exertion or emotion
older adults exam
