What is a PAE?
Where a driver enters a section without the correct authority
What is ‘fit for duty’?
Being free of alcohol/ drugs
Free of fatigue
Free of family or out of work distractions or anything that is impacting your ability to perform job correctly
What is a ‘VIOLATION’?
When a train Driver acts outside of correct rules and operating procedures following a PAE incident
e.g. reversing behind the signal
Resetting the trip & continuing to move forward
What instructions must be followed after a PAE has occurred?
• Immediately bring the train to a stand and secure the train
• Assess the situation in relation to potential risk to other trains
• REC call if Req
• Must not move the train, unless instructed
• Contact controlling signaler or TC & act on the instructions
After a PAE and driver instructed to move train
Driver must then return to FSS or other (as instructed)
To undertake what actions?
• drug/alcohol test
• complete an incident report
• agree to a time for a post PAE interview (next working day rostered)
(Driver stood down pending investigation)
Once driver has had PAE and instructed to drive, do they have to drive the train?
If driver feels unfit to drive they must be relieved
If the cause of a PAE was driver error what plan must be put in place?
Driver Development Plan
Agreed b/w driver & Leader Driver Compliance
Must have clearly defined timescales
What is starting against the signal PAE?
When a driver neglects to check signal prior to departure and assuming it is at proceed
What are some typical errors to starting against signal PAEs?
- Not checking the signal
- Not moving reverser to neutral/ off when sig. @ stop
- moving reverser to FWD/REV without checking signal
- looking at wrong signal
- not controlling distractions
- unable to remember aspect of last signal
What is your STD procedure?
SIGNAL
TIME
DEPARTURE SEQUENCE - (then check mirrors, close doors)
2ND LOOK (@ signal) - then check for steady blue FDCL, whistle, power, release brake)
What is starting on a Yellow?
Having received a yellow warning aspect on approach to the platform, at the end of the platform or after the platform
How can we try to remember when arriving on a Warning aspect when signal ahead (@STOP) is not in clear view?
Press MAD button
How can a driver avoid PAEs when approaching signals?
Use MAD button & reduce speed gradually at the warning signal
Plan to stop well before the STOP signal
Remain aware if have not stopped at that signal before
Use BRAKE if not sure of exact location of the signal
Reduce speed if restricted sighting of signal
Allow for low adhesion or the unexpected
Focus on RED AHEAD
What should you do if you are not sure of next signal location or its aspect?
Recuse speed and approach signal slowly
What are some safety critical errors?
- platform stopping errors
- wrong side door release
- derailments
- train rolling backwards/ forwards with doors released
