PVB L5 Flashcards
What is clinical reasoning ?
Clinical reasoning describes the thinking + decision making processes associated with clinical treatment
It tis the process used by practioners to plan, direct, perform and relfect on client care
Make decisions based on clinical information (history, examinations, tes results) + communicate this effectively with patients / their carers
Outline the principles of clinical thinking
- Use + interpetation of diagnostic tests
- Critical thinking (metacognition)
- Patient - centred evidence based medicine
- Shared decision making
- Clincal skills (history, examination, communication)
- Understanding cognitive biases + human factors
Why is clinical reasoning important
Diagnosis is wrong 10-15% of time
Diagnostic error more likely to lead to harm than other types of error
most diagnostic error is due to error in reasoning, mostly when data not synthesied correctly
MOST DIAGNSOSIS MADE FROM PATIENT HISTORY
What factors impact the reliability of a test
- Normal values
- Factors other than disease which infuence test results
-Operating characteristics
-Sensitivity and specificity - prevalance of dsease in populatuon
What is sensitivity?
The ability to detect true positives
What is specificity?
The ability to detect true negatives
True or False
No test has 100% sensitivity and speficity
True
What is the most fundamental principle in clinical decision making?
Interpretation of new information, depends on what you believed before hand
