Drug Therapy for Obesity
Orlistat (Xenical/alli)
Orlistat: MOA
-OTC medication
-Binds to gastric and pancreatic enzymes and BLOCKS these enzymes; reduces fat absorption by 30%
Orlistat: Side Effects
-Black Box: Liver injury
-GI: Oily spotting, flatulence, and fecal incontinecne
-Decrease vitamin concentration (Must take with multi vitamin)
Teaching Point:
Must take for 3 months to start seeing effect
Drug Therapy Considerations
-Must use with Diet and Exercise
-Best for people with high health risk. (BMI greater than 30) (BMI greater than 27 with risk factors)
-Most doctors will nto prescribe unless patients have tried diet and exercise alone for 6 months
-Limited effectiveness
-Long term treatment (REBOUND)
Other treatment options for Obesity
Bariatric sugery: Liposuction-Gastric Bypass-Grastric Banding
Metabolic Syndrome
-Grouping of risk factors who put someone at risk for CVA and CVD
-Must have 3 factors
Waist Circumference: Men > 40. Women > 35
Triglycerides: > 150mg/dL or drug treatment
HDL cholesterol: <40 mg/dL men. < 50 mg/dL in women
BP: >130 systolic or >85 diastolic
FBG: >110 mg/dL
DRUG TREATMENT CAN SUBSITUTE
