Coagulation Therpay Goals
-Prevent clot formation
-Break apart existing clot
-Help increase circulation and perfusion
-Decrease pain
-Prevent further tissue damage
Biggest Concern
BLEEDING
-All anti-coagulants most common side effect
-Can be internal or external
-Know why your patient is on an anti-coauglant and know what sites they may be at risk for bleeding
-Hemoglobin and hematocrit levels
-Vital signs (Fluid volume deficit) (Tachy is first sign)
Anticoagulants
-Inhibit the action or formation of the clotting factor
-PREVENT clots from forming or from growing bigger
-Does not break one up
Antiplatelet
-Prevent platelet plugs from forming by inhibiting platelet aggregation
-Best for preventing heart attacks and stroke
-Stop platelets from sticking together
Anticoagulants and Antiplatelets are both considered
Antithrombins because they inhibt formation of platelets
Heparin Induced Thrombocytopenia (HIT)
-Low platelet count and increased development of thombi -> caused by antibody development
-Monitor platelet count and stop heparin immediately if platelet count <100,000
-Non-heparin anticoagulants can be used as substitute if anticoagulation still needed
Labs associated with Heparin
-Dose IV heparin based on clotting time labs
-Anti-Xa or aPTT (activated partial thomboplastin time)
-Drawn every 6 hours and draw a baseline before starting heparin
