What type of bacteria is H. pylori?
Gram negative
What does H. pylori cause 90% of?
Duodenal ulcers
What type of ulcers is H. pylori responsible for?
Duodenal and gastric (less so)
How does H. pylori cause a duodenal ulcer?
Buries into gastric epithelium
Neutralises pH of stomach
Damages epithelium
How do you test for H. pylori?
Endoscopy- Rapid urease test on gastric biopsy
What is the criteria for testing for H. pylori?
Patient can’t have taken:
-Anti-biotics or bismuth products for 4 weeks
-PPI for 2 weeks
What is the treatment of a H. pylori infection?
Triple therapy
What is triple therapy?
Amoxicillin/ Metronidazole
Clarithromycin
PPI
What can long term use of a PPI cause?
Hypomagnesaemia (muscle aches)
When should a repeat endoscopy be done?
4-8 weeks
Why is endoscopy repeated after 4-8 weeks?
To ensure that h. pylori has been eradicated
