What is ascites?
Abnormal accumulation of fluid in the peritoneal cavity in the abdomen
How can ascites be diagnosed?
Clinically by looking
What can be done to get a better idea of the cause of ascites?
Ascitic tap
What does SAAG stand for?
Serum ascites albumin gradient
What can the SAAG be used to determine?
The cause of ascites.
How can the SAAG be calculated?
It is calculated by subtracting the albumin concentration of the ascitic fluid from the serum albumin concentration.
What is calssified as a high SAAG?
SAAG > 11g/L
What does an SAAG >11g/L indicate?
Portal hyertension
What are the main hepatic causes of a high SAAG?
Cirrhosis
Budd Chiari syndrome
Hepatic failure
What are the cardiac causes of a high SAAG?
Heart failure
Constrictive pericarditis
What is a low SAAG?
<11g/L
What are the main causes of a low SAAG?
Cancer of the peritoneum
Tuberculosis and other infections
Pancreatitis
Nephrotic syndrome
What should be resticed in someone presenting with ascites?
Reduce dietary sodium
fluid restriction if the sodium is < 125 mmol/L
What medical management can be used in someone presenting with ascites?
Aldosterone antagonists: e.g. spironolactone
What can be used in teh management of recurrent ascites?
Large-volume paracentesis
