Action:
A
- Competes w/ benzodiazepines for binding at the benzodiazepine receptor, reverses the sedative effect of benzodiazepines

Indications:
A
- Benzodiazepine oversedation

Q
Adverse Effects:
A
- Resedation
- Seizures
- Dizziness
- Pain at injection site
- Nausea/vomiting
- Diaphoresis
- Headache
- Visual impairment

Contraindications:
A
- Cyclic antidepressant overdose
- Life threatening conditions that require treatment w/ benzodiazepines, such as status epilepticus and intracranial HTN
- Known sensitivity to Flumazenil/ benzodiazepines
- Use w/ caution where there is the possibility of unrecognized benzodiazepine dependence and in Pts who have a Hx of substance abuse, who are known substance abusers, have a Hx of seizures

Dosage (Adult):
A
- Initial dose is 0.2 mg IV, IO over 15 second period
- If desired dose is not observed after 45 seconds, administer a second 0.2 mg dose over 15 second period
- Doses can be repeated a total of 4 times until a dose of 1 mg has been administered

Dosage (Pediatric):
A
- Children older than 1 year, 0.01 mg/kg IV, IO given over 15 seconds
- May repeat in 45 seconds and then every minute to max dose of 0.05 mg/kg or 1 mg

Special Considerations:
A
- Monitor for signs of hypoventilation and hypoxia for approximately 2 hours
- If the half life of benzodiazepine is longer than Flumazenil, an additional dose may be needed
- May precipitate withdrawal symptoms in Pts dependent on benzodiazepines
- Flumazenil has not been shown to benefit Pts who have overdosed on multiple drugs
