- ACS
- Acute pulmonary embolism
- DVT

- Bleeding
- Thrombocytopenia
- Allergic reactions

Contraindications:

- Predisposition to bleeding
- Aortic aneurysm
- Peptic ulceration
- Known sensitivity
- Hx of heparin-induced thrombocytopenia, severe thrombocytopenia, sulfite sensitivity

Dosage (Adult)
Cardiac Indications:
A
- Initial dose: 30 mg IV
- Then 1 mg/kg every two hours subcutaneously

Cardiac Indications:

- 0.5 to 1.5 mg subcutaneously every 12 hours

- 1 mg/kg every 12 hours
