EKG HR Rule
300, 150, 100, 75, 60, 50
Wandering atrial pacemaker
Changing p waves due to change in atrial depolarization location
A Fib
Fluttering p wave, rapid and random
Irregularly irregular
seen in healthy heart or CAD, hypertension, valvular disease
asymptomatic or mild symptoms (lightheadedness, dyspnea, chest pain)
Atrial tachycardia = supra ventricular tachycardia
Notice increased HR on EKG
Ventricular tachycardia
Sawtooth, regular irregular
Emergency
due to MI or cardiac pathology may lead to vfib
Ventricular fibrillation
Irregular irregular
Emergency
requires CPR
PVC
Irregular placed and larger QRS complex due to disrupted conduction system (wider QRS)
Bigeminy (every other is pvc)
Trigeminy (every third)
Due to anxiety, stress, caffeine, or heart disease
Atrial flutter
Small sawtooth
less severe 250-350 bpm
No p wave
ST segment elevation
Greater than 1 mm indicates MI. emergency
Compare ST to horz line before QRS
ST depression
Greater than 2.0 mm means stop exercise and wait for sx to subside. If they dont treat as emergency
(downsloping and horz)
Upsoping not as strict
1st degree AV Block
PR greater than 0.2
No problems
2nd degree type 1 (Wenckebach)
Progressively longer until drop
2nd degree type 2 (mobitz)
PR is the same each time but random drop
Worse than type 1
3rd degree
Most severe. Stop exercise
Random P waves and QRS, unrelated
Inverted T wave
Myocardial ischemia or LVT hypertrophy due to CHF
