Key imaging modality for multiple sclerosis
MRI
Definition of MS
It is the most common chronic inlfammatory demyelinating disease affecting the CNS of young adults
MS leading in most cases to:
Severe and irreversible clinical disability
Classification of MS according to its clinical presentation:
- Relapsing-remitting (70% of cases)
- 2ry progressive (85%)
- 1ry progressive (10%)
Applications of MRI in MS
• Earlies dx in combination with characteristics and symptoms.
• Understanding of the pathophysiology of MS
• Identify response to therapy in individual patients
MRI sequences used in MS
T1/T1+C
T2
Flair
MS on T1WI:
Lesions are typically iso to hypointense (black holes)
MS on T2 and Flair
Lesions are typically hyperintense
Flair is more sensitive than T2 in detection of:
Juxta-cortical and periventricular plaques
T2 is more sensitive than flair in
Infra-tentorial lesion
Signs of MS
Ependymal-dot
Dawson finger
Ms on T1+C
Active lesions show enhancement (open ring sign)
Other Advanced magnetic resonance techniques that my be used in MS
DWI (Diffusion-weighted image)
MRS (Magnetic resonance spectroscopy)
DIR (Double inversion recovery)
