Primary goal of antilipidemic therapy
Decrease LDL cholesterol
New guideline to lower LDL
Moderate to high intensity statin therapy to decrease atherosclerotic CV risk in adults who have clinical atherosclerotic disease, LDL >190, diabetics aged 40-75
VHDL is a
negative risk factor for coronary artery disease
Statins
HMG-CoA reductase inhibitors
Inhibits rate limiting step in endogenous cholesterol synthesis–liver increases uptake of cholesterol from plasma causing decreased plasma LDL level
Adverse effects of statins
hepatotoxicity
myopathy-muscle pain
Fibrates
PPAR-a agonists, which induce lipoprotein lipase and promote catabolism of VLDL
Examples of fibrates
Gemfibrozil and Fenofibrate
Cholestyramine
Bile acid sequestrant
Decreases intrahepatic cholesterol pool increases LDL receptor gene expression which clears LDL from plasma
Very poorly tolerated due to fat malabsorption
Niacin
Can give in high doses to lower lipids
Statins and pregnancy
May be teratogenic
