What is anaphylaxis?
A severe systemic allergic reaction that is life-threatening and IgE-mediated
Requires immediate treatment.
What are the criteria for diagnosing anaphylaxis?
Acute onset of illness with skin or mucosal signs and symptoms AND at least one of the following:
* Sudden respiratory signs and symptoms
* Sudden systolic BP less than 90 mmHg or symptoms of end-organ dysfunction and/or poor perfusion
OR likely or known exposure with two or more of the following:
* Sudden skin or mucosal signs and symptoms
* Persistent gastrointestinal symptoms
* Sudden respiratory signs and symptoms
* Sudden systolic BP less than 90 mmHg or symptoms of end-organ dysfunction and/or poor perfusion.
What is an anaphylactoid reaction?
A non-IgE mediated reaction that is clinically indistinguishable from anaphylaxis in the prehospital setting
Should be treated identically to anaphylaxis.
What is angioedema?
Abrupt vasodilation and edema of the deep dermal and subcutaneous tissues causing profound swelling, most commonly in the face, tongue, lips, and bowel wall.
What are the common etiologies of angioedema?
- Allergic
- ACE Inhibitor Induced
- Hereditary Angioedema
- Acquired C1 Esterase Deficiency
- Idiopathic.
How should significant mucous membrane swelling be treated in the prehospital setting?
According to the Anaphylaxis Protocol, except for patients with known C1 esterase deficiency on a specialized treatment protocol.
What is the first-line treatment for hypotension in anaphylaxis?
IM epinephrine and IV/IO fluid boluses.
What should be done to treat hypotension in anaphylaxis?
Titrate treatment to a systolic BP greater than 90 mmHg.
What should be done if the patient remains hypotensive despite epinephrine administration and 2L of normal saline?
Mandatory Online Medical Control (OLMC) for further direction.
