7. MTB Step 3 - Urinary Tract Infection Flashcards
CYSTITIS
What are (3) typical Clinical Manifestations of Cystitis?
- Dysuria
- WBC’s urine
- Suprapubic Tenderness
CYSTITIS
What is the typical Demographic of a patient with Cystitis?
Young, otherwise healthy Woman
CYSTITIS
- What is the Best INITIAL Test for Cystitis?
- What is the Most ACCURATE Test for Cystitis?
Best INITIAL Test = Urinalysis
Most ACCURATE Test = Urine Culture
CYSTITIS
What are (2) First-Line Antibiotics that may be used to treat Uncomplicated Cystitis?
Orally for 3 Days:
- Fosfomycin or
- Nitrofurantoin
BOTH are safe in pregnancy
CYSTITIS
What are (2) Antibiotics used to treat Uncomplicated Cystitis if E. coli resistance is > 20%?
- Ciprofloxacin
- Levofloxacin
CYSTITIS
What are (2) First-Line Antibiotics that may be used to treat Complicated Cystitis?
7 Days:
- TMP/SMX (Bactrim) or
- Ciprofloxacin
PYELONEPHRITIS
What are (4) typical Clinical Manifestations of Pyelonephritis?
- Dysuria
- WBC’s urine
- Flank/CV Tenderness
- Fever
Pyelonephritis (higher fever, more ill) is also more severe than Cystitis (lower fever, less ill)
PYELONEPHRITIS
- What is the Best INITIAL Test for Pyelonephritis?
- What is the Most ACCURATE Test for Pyelonephritis?
Best INITIAL Test = Urinalysis
Most ACCURATE Test = Urine Culture
PYELONEPHRITIS
- What is the Antibiotic Treatment for Outpatient Pyelonephritis?
- What are (4) Antibiotic Treatments for Inpatient Pyelonephritis?
Any Medication for Gram-Negative Bacilli:
Outpatient Pyelonephritis
- Ciprofloxacin
Inpatient Pyelonephritis
- Ceftriaxone
- Ertapenem
- Ampicillin + Gentamicin
- Quinolones
CYSTITIS & PYELONEPHRITIS
When are Sonography and CT Scanning used for Cystitis & Pyelonephritis?
To determine Etiology of UTI (not to diagnose UTI)
-
Complicated
- Stone
- Stricture
- Tumor
- Obstruction
CYSTITIS & PYELONEPHRITIS
- On Urinalysis, what information does a Positive Leukocyte Esterase test give you?
- On Urinalysis, what information does a Positive Nitrite test give you?
- Leukocyte Esterase is derived from granulocytic WBCs and serves as Indirect evidence of the presence of Bacteriuria.
- Nitrites are indicative of Gram-Negative Bacteria. Protein is very nonspecific in a urinalysis. Protein can be from both infection and glomerular disorders. Red cells are nonspecific as well.
PERINEPHRIC ABSCESS
This is a Rare Complication of which UTI?
Pyelonephritis
PERINEPHRIC ABSCESS
What is the typical Clinical Manifestation of a patient with a Perinephric Abscess?
A patient with Pyelonephritis (dysuria+wbc+flank pain+fever) who does NOT respond to treatment after 5 - 7 days.
Patient remains Febrile and still shows WBCs on Urinalysis.
PERINEPHRIC ABSCESS
- What are (2) Best INITIAL Tests for Perinephric Abscess?
- What is the ONLY way to determine a precise Microbiologic Diagnosis to guide therapy for Perinephric Abscess?
- Best INITIAL Test = Sonogram or CT Scan
- _Microbiologic D_x = Biopsy
PERINEPHRIC ABSCESS
What is the Treatment for Perinephric Abscess?
Quinolone + Staphylococcal coverage (eg, oxacillin, nafcillin, vancomycin)
because treatment with antibiotics for gram-negative organisms preferentially selects out for staphylococci
