reason for rejection, and another assignment or reasoning why light duty will not be offered
If a commander approves a light duty request what shall the requesting supervisor do?
Immediately provide a BFOE
After a BFOE is signed what shall a supervisor EMAIL and to who?
DWC Form 73 or phsyician statement and signed BFOE. HSU
After a BFOE is signed what shall a supervisor interoffice mail and to who?
original BFOE, original DWC 73 Form, copy of FMLA correspondence accepting or declining, sent to HSU
What shall the commander do once approval for light duty occurs?
Notify time keeper, add injured employee to division Injured or Ill Employees report
How long normally are light duty assignments allowed for?
90 days
Who shall be contacted if light duty status changes of an employee?
HSU
When on light duty what is a employee responsible for every 30 days or after every physician visit?
Updated DWC Form 73
Where may a classified employee be offered a light duty assignment?
Their own unit, current command, Teleserve, any other dept with approval of AC
Where may a civilian employee be offered a light duty assignment?
Their own unit, current command, other dept with approval of AC
What additionally shall happen if a spot is not available for a light duty employee in their own division?
Notify HSU
May light duty employees wear their uniform or display ID Card?
No, except in Police facility
How long may a civilian request for light duty assignment?
90 at first and then additional 90 for 180 total
