week 5 - wound managment Flashcards
what are semipermiblae adhesive films?
tegaderms- permiable to gas and water vapur. barrier to bacteri nad water. can be left in place for several days
what are hydrocolloid dressings?
adhesive and are impermeable to gas and water. when the inner layer becomes into contact wth exudates it forms a gel. helps to debride the wound. wounds can appear to swell in size and start to smell
dressing in place for 5-6 days
what are alginate wound dressings?
derived from seweed
they are activated by exudates to produce a hydrophillic gel. generally not good if there is nercrotic tissue. Good for deep wounds with dead tissue as it draws it out
should be changed once or twice a week
what are hydrogel wound dressing?
provide moisture- should be used on dry necrotic wounds, absorbs exudate
what are the types of anesthsia to help reduce pain when repairin?G
topical
Local - direct infiltration, regional block
when should glues be used to close a wound?
small lacerations - smaller time to close tissue. must be cleansed and dry fisrt and must have heamostasis
not for use in high tension areas
the glue will sloth off in 5-10 days and is realaity fast and painless to apply
how do you remove a forgine body from a wound?
visual inspection- imaging if needed
discuss and document any retained forign body
how do you irrigate a wound?
local anesthsia prior to irrigation
dont soak would
use normal saline or tap water
large syrige with butterfly or stylet attached
dont use iodine
how do you debride a wound?
remove forgine matter and devitalised tissue
create shrap wound edges
respect skin lines
when should staples be used?
used to replace sutures
better in head or neck as they offer 10 times faster closure and have similar results in comfort and recovory rate
what is key to skin closure?
wound edge eversion
approximate
anticipate odeama
what are the different types of suturing techneques?
simple interupted
simple continuous
horozontal matteress
what local anethetics do we use?
lignocian and adrenalin- no adrenalin in fingers due to risk of necrosis
