Urinary Incontinence (Repro) Flashcards
Overactive bladder syndrome is defined as?
Urgency that occurs with or without urgency incontinence and usually with frequency and nocturia
Overactive bladder syndrome is defined as?
Urgency that occurs with or without urgency incontinence and usually with frequency and nocturia
What is characteristic of OAB?
Urinary frequency
Only small voided volumes
What is OAB a form of?
Bladder dysfunction characterized by detrusor contractions during the inhibition of voiding phase
Most common cause of urge incontinence and OAB?
Idiopathic detrusor overactivity (in middle aged females)
Another common cause of urge incontinence?
Atrophic vaginitis
Treatment of atrophic vaginitis?
Topical oestrogens
Treatment of urinary incontinence?
- Lifestyle advice
- Bladder drill
- Medication
- Botox
- Neuromodulation
- Reconstructive surgery
Medications for treating urinary incontinence?
- Anti-muscarinics
- Beta agonists
What do anti-muscarinics do to terat urinary incontinence?
Block PS to stop detrusor contractions
Examples of anti-muscarinics?
Solifenacin
Beta agonists work by?
Activating sympathetics to increase detrusor relaxation
Examples of beta agonists?
Mirabegron
What is stress incontinence?
Involuntray leakage of urine of effort, exertion, sneezing or coughing causing increased intra-abdominal pressure without detrsor contraction
What is stress incontinence due to?
Damage or weakness of the pelvic floor/urethral function
RFs for stress incontinence?
- Pregnancies
- Childbirth
- Menopause
- Increased age
- Obesity
- Smoking (chronic cough)
Investigation for stress incontinence?
Standing or supine stress test where you ask the ptnt to cough and look for leakage of urine
Treatment of stress incontinence?
Lifestyle advice Pelvic floor physio Pessaries Medication Incontinence pads Surgery
Medications for stress incontinence?
Duloxetine which is a combined NA and SSRI thought to increase intraurethral closure pressure
Why does overflow incontinence occur?
Due to bladder not emptying properly when urinating causing urine to leak out later
Which incontinence is more common in men?
Overflow incontinence
Due to prostatic obstruction
Reasons overflow incontinence can happen in women?
Obstruction of the urethra (caused by prolapse)
Poor contractile bladder muscle
Presentation of overflow incontinence?
Painful urinary retention
-Palpable bladder, chronic retention, wet and nigh and renal impairment, mass is due to percussion
What must be stopped if someone presents with overflow incontinence/acute retention?
Anti-cholinergics
Anti-muscarinics
Patients with overflow incontinence have full recoveries. True or False?
FALSE
Unlikely to ever be able to pass urine spontaneously again
(often people need self-catheterisation)
