What is puerperal psychosis?
An emergency and requires same day assessment by psychiatry if suspected
-usually presents within 2 weeks of delivery
Early symptoms puerperal psychosis?
Sleep disturbance confusion Irrational ideas Progresses to 5% suicide risk Hallucinations
RF for development is puerperal psychosis?
Go into develop bipolar disorder
Previous episode of puerperal psychosis
1st degree relative with bipolar
Treatment of peurperal psychosis?
Antipsychotics
Anti-Depressants
Mood stabilisers
Postnatal depression?
2-6 weeks postnatal and lasts weeks to months
Anti-depressants in pregnancy?
SSRI= first line: sertraline
Antipsychotics in pregnant?
2nd generation drugs: olanzapine and quetiapine
Pregnancy and bi-polar medication?
Switch to maybe queriapine
Increase monitoring is needed if lithium required
Lithium in pregnancy?
Should be avoided
Assoc with cardiac abnormalities: Ebstein’s anomaly
Anxiety meds in pregnancy?
SSRIs: first line
Benzos: In 3rd trimester can = floppy baby so generally avoid
What antipsychotic to avoid in pregnancy?
Clozapine (agranulocytosis)
Don’t breast feed with what drugs?
Lithium
Valproate
