What is chlamydia?
Most comonly reported bacetrial STI in sexual health clinics
Where does chlamydia infect?
Columnar epithelium at mucosal sites
Transmission of chlamydia?
Oral, vaginal or anal sex
What age is common to get chlamydia?
20-24 years
What % of women with chlamydia develop PID?
9%
Presentation of chlamydia?
- Milky urethral discharge in men
- Irregular bleeding
- Abdominal pain
- Dysuria men
- Urethritis
- Cervicitis
- Epididymo-orchitis
- Proctitis (LGV)
Testing for chlamydia?
Vulvovaginal swab (GC/CT NAAT) pre speculum
Urine: men (GC/CT NAAT 1st void urine)
Throat swab: GC/CT NAAT
Rectal swab: GC/CT NAAT
Testing advice for chlamydia?
- Test 14 days following exposure
- Vaginal discharge on its own is of poor predicative value in women >25 y/o
Prognosis of chlamydia?
1/5 women diagnosed and treated are estimated to become re-infected within 10 months after initial treatment
Treatment of chlamydia?
Doxycycline 100mg BD 1 week
Azithromycin 1G stat followed by 500mg daily for 2 days
Treatment of PID after chlamydia?
Ceftriazone 1G, Doxycycline 100mg BD x 2 weeks and metrondiazole 400mg BD x 2 weeks
Complications of chlamydia?
PID
Ectopic pregnancy
Conjunctivitis
Fitz hugh Curtis
Gram staining and chlamydia?
Doesn’t work because n peptidoglycan in cell wall
Serovars D-K causes what?
Genital infection
Serovars A-C causes?
Eye infection
