How is PCP different from bacterial PNA?
- non-productive cough
- no consolidations on CXR
- ground-glass appearance on CXR
Cue:
- ping pong table is made of glass but is cracked and being held up by silver discs
- PCP = ground glass appearance on CXR
- silver = methamine silver stain
- disc = disc-shaped yeast
What is the tx for PCP?
- Bactrim (Trimethoprim-Sulfamethoxazole)
- Pentamadine (if sulfa allergy)
Cue:
- player squirting water into his mouth with a water bottle labeled “BAL”
bronchoalveolar lavage (BAL) to diagnose PCP
Cue:
- ping pong player with a cane
- ping pong (PP) = Pneumocystic pneumonia (PCP)
- cane = immunocompromised
Cue:
- backhand in front of a jar of ping-pong balls shaped like eggs and pentagon-shaped paddles
- tx for PCP:
- backhand = Bactrim
- eggs = sulfur = sulfa drugs (TMP-SMX)
- pentagon = Pentamidine

How is the PCP diagnosis confirmed?
bronchoalveolar lavage (BAL) –> methamine silver stain of sample –> disk-shaped yeasts

- “Aid for AIDS” sign
Pneumocystis jiroveci = associated with AIDS
Cue:
- scoreboard shows 20-0
20-0 = 200 = CD4+ less than 200 for PCP
