PRALIDOXIME- MoA
Reactivate acetylcholinesterase to effectively act as an antidote in organophosphate and other pesticide poisionings
Allows for acetylcholine (Ach) to be destroyed at the neuromuscular junction resulting in the reversal of muscle paralysis.

PRALIDOXIME- Contra
Use a reduced dose in myasthenia gravis patients and in those with impaired renal function.
PRALIDOXIME- S/E
**Cardiovascular: **tachycardia, cardiac arrest hyperventilation,
CNS: neuromuscular blockade, headache, dizziness, drowsiness, seizure
Respiratory: apnea, laryngospasm,
Other: muscle rigidity/weakness, nausea, rash, pain at injection site
PRALIDOXIME- Routes
IVPB, IM, SC
PRALIDOXIME- A. Dose
Organophosphate poisoning:
1-2 g IVPB over 30-60 minutes, after Atropine administration.
Can repeat in 60 minutes if paralysis Persists.
600 mg IM/SC repeat every 15 mins x 2 if needed.
Acetylcholinesterase OD:
1-2 g IVPB over 30-60 minutes.
Can Repeat at 250 mg every 5 minutes as needed for paralysis.
PRALIDOXIME- P. Dose
20-40 mg/kg IVPB over 15-30 minutes. Dose can be repeated in 1 hour if muscle paralysis still remains. Can use IM/SC admin if no vascular access available.
PRALIDOXIME- EXTRAS
Precautions: Pregnancy Category C Slow IV push limits cardiovascular side effects. Rapid IV infusion may cause muscle rigidity, tachycardia, or laryngospasm. Neonates should not receive high concentrations for long periods of time Interactions: Do not use with succinylcholine, morphine, aminophylline, theophylline, or other respiratory depressants like barbiturates, benzodiazepines, narcotics, and other sedative hypnotics. Onset in 1-2 minutesl; duration of action is unknown Treatment is most beneficial when done with a few hours of exposure.
