Selective serotonin (5-HT3) receptor antagonist, antiemetic
Ondansetron- MoA
Selective antagonist of the neurotransmitter serotonin (5HT3) in the CNS at the chemoreceptor trigger zone and in the peripheral nervous system on nerve terminals of the vagus nerve – blocking nausea and emesis.

Congenital long QT syndrome
Ondansetron- S/E
CNS: dizziness, fatigue, headache, malaise, sedation, extrapyramidal reactions
Cardiovascular: arrhythmias, chest pain
GI: constipation, diarrhea, abdominal pain
Other: musculoskeletal pain, chills
Ondansetron- Routes
IV, IO, Rectal, PO
(also given in orally disintegrating tablets (ODT)
Ondansetron- A. Dose
(vary based upon source of nausea and vomiting but average what is listed below)
4 mg IV push, undiluted, over 2-5 minutes
Ondansetron- P. Dose
(vary based upon source of nausea and vomiting but average what is listed below)
Children (1 month to 12 years who weigh more than 40kg):
4 mg IV, undiluted over 2-5 minutes
Children (1 month to 12 years who weigh less than 40 kg):
0.1 mg/kg IV as a single dose
Ondansetron- EXTRAS
Precautions: Closely monitor patients with hypokalemia, hypomagnesemia, CHF, and bradycardia, Zofran may prolong the QT interval and lead to fatal heart rhythms, including Torsades. Do not utilize the ODT in patients with a history of PKU (phenylketonuria). Interactions: Drugs such as cimetidine (Tagamet) that alter hepatic drug-metabolizing enzymes, Phenobarbital and rifampin (antibiotic) may change onset and duration of action of Zofran. Cannot be given through same IV as furosemide, lorazepam, or sodium bicarbonate. Onset and Duration of Action: all unknown Dosages: (vary based upon source of nausea and vomiting but average what is listed below) Now being prescribed increasingly often to manage N/V related to pregnancy and/or acute gastroenteritis.
