Nitrate vasodilator
Antidote
AMYL NITRITE- MoA
Relaxes vascular smooth muscle, decreasing vascular resistance (decreases preload and afterload)
Converts hemoglobin to methemoglobin, which then binds with cyanide to form non-toxic cyanomethemoglobin that can be excreted
AMYL NITRITE- Indication
Angina Hypertension
Cyanide poisoning
AMYL NITRITE- Contra
Hypotension
Erectile Dysfunction (PDE 5 inhibitors) medications within the last 36 hours
No contraindications when being used to treat cyanide poisoning
AMYL NITRITE- S/E
Cardiovascular: hypotension, tachycardia, palpitations, cardiovascular collapse
CNS: headache, dizziness, weakness
Respiratory: respiratory depression
AMYL NITRITE- Routes
Inhalation
AMYL NITRITE- A. Dose
1-2 ampules (0.3 ml),
crushed and inhaled over 30 seconds
May repeat every 1-2 minutes
AMYL NITRITE- P. Dose
1 ampule (0.3 ml),
crushed and inhaled for 15-30 seconds
May repeat every 1-2 minutes
AMYL NITRITE- EXTRAS
Precautions:
Pregnancy
Shock
Tendency for abuse
Interactions:
Hypotensive effects will be potentiated with other vasodilators such as ACE-inhibitors, beta blockers, narcotics, certain phenothiazines and other nitrates
Onset in 10-30 seconds and may last 3-5 minutes
Notes: High flow oxygen should be administered in between doses
