Parasite II Flashcards
chronic symptom of schistosoma japonicum and mansoni
symmer’s pipe stem fibrosis (hepatic granuloma/fibrosis)
acute symptoms of schistosoma
- history of recent contact with contaminated water
- swimmer’s itch - due to penetration of schistosonium (0-3 days)
- sschistosomular pnuemonitis - migration of schist into lungs (3-10 days)
- acute schistosomiasis - katayama fever –> acute febrile reaction with hypereosinophilia
treatment of schistosoma
praziquantel
what the three phases of trichinella
enteric phase, migratory (invasive) phase, and encystment phase
what is seen in enteric phase of trichinella
most cases – it is mild
nausea, vomiting, diarrhea, colic, sweating
what is seen in the migratory (invasive) phase of trichinella
peri orbital edema, eosinophilia, myalgia
what is seen in the encystment phase
fever, edema, dehydration usually subsides, eosinophilia subsides and remains elevated, elevated IgE antibodies
treatment of trichinella
mebendazole
treatment for loa loa
Albendazole, Ivermectin, DEC
LAID
control of loa loa
DEC prophylaxis
wearing light colored trousers
fly screens
Insect repellents
lifecycle of simulium
L1 microfilariae in skin and when bug bites it picks up L1 –> bug turn L1 to L3 –> bites person again inoculating them with the L3 larvae –> onchocerca volvulus
diagnosis of onchocerca volvulus
bloodless skin snips
symptoms of onchocerca volvulus
papular dermatitis, hanging groin (loss of elasticity), onchocercomata (looks like bump on head from adult worms), skin atrophy, depigmentation, lichenification (lizard skin), sclerosing keratitis –> blindness
main cause of inflammation in onchocerca volvulus and what is it mediated by
wolbachia mediated by TLR 4
treatment of onchocerca volvulus and what they target
ivermectin –> microfilaricide (does not work on the wolbachia bacteria)
doxycycline –> kills wolbachia bacteria (adult worm)
