a. Any Fatal or Severe collision involving ____ or________ employees occurring within ANY jurisdiction will be invetigated by MAIT in conjunction with local jurisdiction.
b. Any accident involving a fatal or severe injury, or substantial damage involving a departmental _________ or when specifically requested by Office of AIR Operations to assist with an aircraft collision investigation.
c. Any other collision of unusual significance, subject tp the prior approval of the team’s respective Division CDR.
