Chapter 2 (Part 5) Flashcards
Upper Respiratory Problems
- Rhinitis
a. Allergic Rhinitis
b. Acute/Viral Rhinitis - Sinusitis
- Influenza
- Tonsilitis/Adenoiditis
- Pharyngitis
- Laryngitis
- Epistaxis
- Obstructive Sleep Apnea
A group of disorders characterized by inflammation and irritation of the mucous membrane of the nose
Rhinitis
It is a non-specific term that covers
infections, allergies, and other disorders
whose common feature is the location at their symptoms
Coryza
Non-allergic factors (Rhinitis)
- Environmental Factors
- Changes in temperature or humidity
- Odors, foods, infection
- OTC medicines
- Presence of foreign bodies
Allergic rhinitis sometimes called
Hay fever
It is the reaction of the nasal mucosa to a specific antigen (allergen) caused by allergy to pollens from trees, flowers, or gasses.
Allergic Rhinitis
Inflammatory cells
- Neutrophils
- Eosinophils
- Lymphocytes
Diagnostic Test (Allergic Rhinitis)
- Allergy skin test
- Fluorescent enzyme immunoassay (FEIA)
- WBC
Intervention for Allergic Rhinitis
- Adequate fluid intake
- Rest
- Warm salt water gargle
- Proper hand washing
- Keeping warm
- Give medications
- Humidifiers, mist, nebulizer
Usually caused by a viral or bacterial infection
Acute viral/bacterial rhinitis
often is used when referring to a URI that is self limited and caused by a virus.
Viral rhinitis (Common cold)
refers to an infectious, acute inflammation of the mucous membranes of the nasal cavity characterized by nasal congestion, rhinorrhea, sneezing, sore throat, and general malaise.
Cold
Nursing management of Acute Rhinitis
- Instruct pt to avoid or reduce exposure to allergens or irritants
- Pt education ragarding the use of OTC medications
- Saline nasal spray
Medical Management for acute rhinitis
- Anti-histamine
- Decongestant
- Low dose steroids therapy/intranasal
- Corticosteroids
- Antibiotics
Is an inflammation of the mucous
membrane of the paranasal sinuses
Sinusitis
Acute sinusitis
less than 4 weeks
Sub-acute sinusitis
4 to 12 weeks
Chronic sinusitis
greater than 12 weeks
is the infection of more than one sinus.
Parasinusitis
is an inflammation of the paranasal sinuses nasal cavity (nose and sinuses)
Rhinosinusitis
90% of sinusitis
Viral infection
Bacteria for sinusitis
Streptococcus Pneumoniae
Complications for Sinusitis
Meningitis
Brain abscess
Osteomyelitis
Ischemic Brain infarction
Clinical manifestation of Sinusitis
- Facial Pain
- Nasal obstruction
- Fatigue
- Purulent nasal discharge
- Fever, headache
- Ear pain
- Cough early mucous
Diagnosis for sinusitis
- Sinus xray
- ct scan
- sinus aspirate through culture technique
Medical Management for Sinusitis
- Antibiotics
- Decongestant
- Antihistamines
- Healed mist or saline irrigation
Nursing management for sinusitis
- Pt education
- Instruct pt about methods to promote drainage of the sinuses
- advise pt to avoid swimming, diving, travel
- Stresses the importance of following recommended antibiotic regimens
- Explain that fever, severe headache, and nuchal rigidity are signs of complication
Viral infection of the respiratory tract
Influenza
A communicable disease spread by droplet infection differs from a common cold primarily in its sudden onset
Influenza
Incubation period of influenza
1-4 days
Manifestations of influenza
- Abrupt onset of chills and fever
- Fever
- malaise
- Muscle aches
- headaches
Complications of influenza
- Sinusitis
- Pneumonia
- Otitis media
Out pouching of mucous membrane lining the nose or paranasal sinuses
Nasal Polyps
Assessment of nasal polyps
- Revealed in Gross examination
- Nasal obstruction
Removal of polyps
Nasal Polypectomy
Acute infection of palatine tonsils commonly caused by streptococcal organism
Tonsilitis/Adenoiditis
Signs and symptoms /Manifestations of Tonsilitis/Adenoiditis
- Sore throat
- fever
- snoring
- Difficulty in swallowing
- Mouth breathing
- Earache
- Draining ears
- halitosis
- General body malaise
- Cervical lymphadenopathy
Diagnostic test for Tonsilitis/Adenoiditis
- rapid antigen detection test (RADT)
- Rapid strep test (RST)
- Culture of tonsillar Site
Surgical treatment for Tonsilitis/Adenoiditis
Tonsillectomy if the pt had multiple tonsilitis in a year
Complication of Tonsilitis/Adenoiditis
- Acute otitis and acute mastoiditis
- AGN
- RHF & RHD
- Pneumonia
- Osteomyelitis
- Hemorrhage (Post-op)
Nursing management for Tonsilitis/Adenoiditis
- Increase fluid intake
- Administer analgesic
- Salt water gargle
- rest
- give penicillin for bacterial infections
- avoid vigorous toothbrushing
