Q

What is there low PTH but high calcium in hypercalcemia of malignancy?

A

No parallel rise in phosphate concentration • No rise in plasma alkaline phosphatase (usually seen due to osteoclast activity) – Humoral hypercalcaemia of malignancy is caused by synthesis of Parathyroid Hormone-Related Peptide (PTHrP) • Secreted in breast and certain lung tumours • Not detected by PTH assay
