Antiplatelet agents Flashcards
MOA of aspirin?
Inhibition of platelet COX-1 (irreversible)
What 4 drugs platelet ADP (P2Y12) receptors?
Which are reversible? Irreversible?
Ticagrelor - reversible
Clopidogrel and Prasugrel - irreversible.
MOA of dipyridamole?
Inhibits phosphodiesterase in plateles, increasing cAMP, which increases PGI2 causing an anti-aggregation effect
MOA of abciximab, eptifibatide, and tirofiban?
Blocks platelet GIIb and IIIa receptor, preventing fibrinogen binding and platelet aggregation.
Abciximab works via monoclonal antibody
Eptifibatide works via cyclic peptide
Tirofiban works via non-peptide
What platelet ADP receptor inhibitor is less efficacious than the others?
Clopidogrel is less efficacious than ticagrelor or prasugrel.
Onset of action of aspirin?
rapid
Onset of action of ticagrelor, clopidogrel, and prasugrel?
2 hours
Elimination of aspirin?
Hepatic
Elimination of clopidogrel?
Renal and fecal excretion
Elimination of dipyridamole?
Hepatic phase II conjugation
Adverse reactions to aspirin?
Gastric upset
Bleeding
DDI occur between clopidogrel and ?
PPIs
Uses for aspirin?
Acute MI, Unstable angina, PCI, secondary prevention of MI/stroke
Uses for clopidogrel?
Acute MI, unstable angina, PCI
Uses for dipyridamole?
Secondary prevention of MI/stroke
