Anatomy of O&G Incisions Flashcards
What are Langer lines?
pattern of lines following collagen fibres
What ribs does EO attach to?
5-12
What are the attachments of EO?
ribs; iliac crest; pubic tubercle and linea alba
What is the linea alba?
midline blending of aponeuroses
What is formed by the inferior free edge of the EO?
inguinal ligmanet
At what point does the EO become aponeurotic apporiximately?
midclavicular line
What are the attachemnts of IO?
lower ribs- 10-12; thoracolumbar fascia; iliac crest and linea alba
What are the attachemnts of TA?
lower ribs; thoracolumbar fascia; iliac crest and linea alba
What divides the rectus abdominus?
tendinous intersections
What is the function of hte tendinous intersections of TA?
improved mechanical effiency
What does the linea alba run between?
xiphoid process to pubic sympysis
What is the rectus sheath?
combined aponeuroses of anterolateral abdo wall muscles
What is found in the space between TA and IO?
all NVBs to anterolateral abdo wall run
Where do the NVBs in the thorax run?
between internal and innermost intercostal muscles
What is the function of pyramidalis?
tenses linea alba
What is the attachment of pyramidalis?
pubic bones to linea alba
Where does the arcuate line lie?
13rd of wat between umbilicaus and pubic crest
What is the difference between the rectus sheath above and below the arcuate line?
above- anterior and posterior rectus sheath; below- just antieorr rectus sheath
What forms the anteiror rectus sheath above the arcuate line?
EO and IO
What forms the posteiror rectus sheath above the arcuate line?
IO and TA
What forms the anteiror rectus sheath below the arcuate line?
EO; IO and TA
What is foudn deep to TA?
transversalis fascia; extraperitoneal fat and peritoneal peritoneum
What is the nerve supply to the anterolateral abdo wall?
thoracoabdominal nerves; subcostal; iliohyogastric and ilioinguinal
What nerves form the thoracoabdominal nerves?
7-11th intercostal nerves
What is the nerve root of subcostal?
T12
What artery are the superior epigastric arteries a continuation of?
internal thoracic
Where do the epigastric arteries lie?
posterior to rectus abdominus
What are the inferior epigastric arteries a branch of?
external iliac artery
What arteries supply the anteiror abdo wall?
sup. and inf. epigastric arteries
Where do the epigastric arteries anastamose?
arounnd the umbilicus
What arteries supply the lateral abdominal wall?
intercostal and subcostal arteries
What are the intercostal nad subcostal arteries branches of?
posteiror intercostal arteies
How is trauma to muscle fibres minimised when making incisions?
incising in same direction as muscle fibre
What is the problem with a midline incision for laparotomy?
relatively bloodless which increases chance of wound complications
What should be avoided when making a lateral port for laparscopy?
inferior epiastric artery
Where does the inferior epigastric artery emerge?
just medial to the deep inguinal ring
What direction does the inferior epigastric artery travel?
superomedially
