Anatomy of Anaesthesia in Labour Flashcards
What nerves supply the pelvis generally?
sympathetic, parasympathetic and visceral afferent
What nerves supply structures in the perineum generally?
somatic motor and somatic sensory
What nerves supply organs touching the peritoneum, and what nerves do these run along with?
visceral afferents running alongside sympathetics
What nerves supply organs not touching the peritoneum and what nerves do these run along with?
visceral afferents running alongside parasympathetics
What spinal levels do visceral afferents from structures touching the peritoneum?
T11-L2
What spinal levels do visceral afferents not touching the peritoneum enter at?
S2-4
What structures does the pudendal nerve take sensory info from?
inferior vagina; perineal muscles; glands and skin
Where does the spinal cord become the cauda equina?
L2
What level does the subarachnoid space end?
S2
Where is anaesthetic in spinals and epidurals injected?
L3/4
What ligaments does the needle for a spinal enter through?
supraspinous; interspinous and ligamentum flavum
What is the function of hte supraspinous?
runs over the spinous processes
What is the function of interspinous?
runs inbetween spinous processes
What is the function of ligamentum flavum?
connects the lamina of adjacent bodies
Where do sympathetic nerves exit the spinal cord?
T1-L2
How do sympathetic ganglia recieve fibres below L2?
via the sympathetic chain and distribute via lumbar; sacral and coccygeal spinal nerves
What vessels do sympathetic fibres supply?
all arterioles
What does bloackade of sympethic tone cause in vessels?
vasodilation
What are the results ofvasodilation due to sympathetic blockade?
skin lookds flushed; warmer and reduced sweating; hypotension
What is the pudendal canal?
passageway within obturator fascia
What travels with the pudendal nerve in the pudendal canal?
internal pudendal a. and v. and nerve to obturator internus
How does pudendal nerve travel ?
exits pelvis via greater sciatic foramen; passes posterior to sacrospinous ligament- lateral to ischial spine; re-enters pelvis/perineum via lesser sciatic foraen
What is an episiotomy?
made from the posterior aspect of the vaginal opening; posterolateral incision into relatively safe fat-filled ischioanal fossa to avoid incision extending into rectum
