Verapamil - Side Effects

A
  • HoTN (transient)
  • Asytole
  • Bradycardias, including heart blocks
  • May increase ventricular rate in some cases of bypass tract

  • Congestive heart failure
  • WPW syndrome
  • Heart blocks
  • Wide complex tachycardias

Verapamil - Special Information

A
  • Prior to admin, consider pretreatment with 5 ml Calcium Chloride 10% to prevent HoTN
  • Adenosine is first med of choice for SVT per AHA ACLS guidelines
  • Can slow rate in rapid A-fib
  • Elderly pts on calcium channel blockers may require lower dosage
  • Discontinue admin at first sign of slowing heart rate
  • Can induce VF resistant to treatment in the pt with WPW syndrome. The ECG may show WPW as a shortened PRI (0.12 sec) - consider lidocaine to treat these pts
  • Admin of verapamil to pt with wide complex tachycardia can be a LETHAL ERROR!!!
  • Monitor pulse rate,ECG, and BP closely
