- Narcotic Analgesic
* CNS Depesant

Fentanyl - Actions
A
- CNS depression - binds to opiate receptor sites in the CNS
* Alters the response to and perception of pain

Fentanyl - Indications
A
- Sedation prior to intubation
* Moderate to severe pain (CP, burns, isolated extremity trauma)

Fentanyl - Dosage/Route
A
- 25-100 mcg slow IVP (over 2-3 min)
* May repeat in 30 min. prn

Fentanyl - Side Effects
A
- Decreased LOC
- Paradoxical excitation
- Respiratory depression &/or arrest
- Arrhythmias, bradycardia, hypotension
- N/V

Fentanyl - Contraindications
A
- Known hypersensitivity to the drug
- MAO Inhibitors within 14 days
- Myasthenia Gravis

Fentanyl - Special Information
A
- May produce severe or fatal reactions when given to pts on MAOI’s
- Additive CNS depression with alcohol, antidepressants, antihistamines, or sedative/hypnotics
- Monitor VS & ECG continuously
- Naloxone will reverse overdose effects
- Assess effectiveness of administration with reassessment of pain, respiratory status & sedative effects
- Use with caution in pts with head trauma, increased ICP, undiagnosed abdominal pain, elderly, debilitated
- Reduce dosage for pts with COPD, respiratory problems, or hepatic/renal insufficiency
