A
- Relaxes vascular smooth muscle & slows conduction through the AV node
- Slows the rapid ventricular rate associated with A-fib & A-flutter

Diltiazem - Indications
A
- Rapid ventricular rate in A-fib & A-flutter
- Angina
- After Adenosine for refractory PSVT with narrow QRS complex & adequate BP

Diltiazem - Dosage/Route
A
- 15 - 20 mg (0.25 mg/kg) slow IVP over 2 min
* May repeat in 15 min with 20 - 25 mg (0.35 mg/kg) slow IVP over 2 min

A
- Dizziness/Headache
- Bradycardia/AV Blocks/Asystole
- Hypotension
- N/V

Diltiazem - Contraindications
A
- Wide QRS Tachycardias of uncertain origin
- Poison/Drug induced Tachycardias
- Sick Sinus Syndrome/AV Blocks
- AMI & Pulmonary Congestion
- WPW with rapid A-fib/A-flutter

Diltiazem - Special Information
A
- Monitor BP frequently
- Consider fluid bolus & Calcium Gluconate for severe Diltialzem-induced HoTN
- Drug needs to be kept refrigerated
