  • Shortens the refractory period of accessory pathways
  • Acts as a negative inotrope and vasodilator

VERAPAMIL Indications

A
  • Stable narrow-QRS tachycardia if the rhythm persists despite vagal maneuvers or adenosine
  • To control the ventricular rate in patients with A-fib or A-flutter without preexcitation

VERAPAMIL Adult dose

A
  • Initial dose:
    • 2.5 – 5 mg IV/IO SLOWLY (over 2 min., 3 min. in elderly) titrate for effect
  • Repeat dose:
    may repeat in 15-30 min. as needed with
    • 5–10mg IV/IO SLOWLY (over 2 – 3 min.) titrate for effect max total dose of 20 mg
  • Alternative dose:
    • 5 mg IV/IO SLOWLY every 15 min. until desired effect obtained or max total dose of 30 mg

VERAPAMIL Contraindications

A
  • Known hypersensitivity
  • Wolff-Parkinson-White or Lown-Ganong-Levine syndromes
  • 2nd or 3rd AV heart blocks
  • Sick sinus syndrome or other SA node disease unless functioning pacemaker is present
  • Hypotension
  • Cardiogenic shock
  • Severe left-ventricular dysfunction
  • Wide-complex tachycardias
  • Children younger than 1 year
  • Use with caution in patients taking oral beta-blockers
