Warfarin INR 2-3: A valve
Warfarin INR 2.5-3.5: M valve or A valve w/ RFs
Catscratch disease presentation; tx of it
papule at scratch/bite site, regional adenopathy, +/- fever
tx = azithromycin; may add in clinda if picture unclear
Patient has kidney disease, increased bleeding time, normal PT/PTT/plt’s, and an active bleed. Next step?
Give desmopressin (increases release of factor 8:VWF multimers)
when parathyroidectomy is indicated in 3* hyperparathyroidism
persistent hyperCa or hyperP, very high PTH, soft tissue calcification or calciphylaxis, intractable bone pain
tx of psychosis in Parkinson’s patient who has already had multiple adjustments of their dopaminergic meds
quetiapine, clozapine, or pimavanserin (serotonin 5-HT-2A receptor inverse agonist)
stimulant toxicity presentation
complaints: anxiety, agitation, insomnia, loss of appetite
PE: tachycardia, HTN, sweaty, mydriasis
possible psychosis and seizures
difference btwn diphenhydramiine toxicity versus stimulant toxicity
They look very similar but diphenhydramine toxicity would also have dry skin, ileus, and/or urinary retention.
