Pharmacology and presentation of Sodium Bicarbonate 8.4%?

100ml Glass Bottle
Sodium Bicarbonate is a hypertonic solution that acts as a buffer. Excess serum hydrogen ions react with the bicarbonate resulting in the formation of carbon dioxide and water. This action assists in restoring pH to within normal ranges.

Indicators of Sodium Bicarbonate 8.4%?
- CCHT
A
- Tricyclic Antidepressant Overdose (TCA OD)
- Cardiac arrest secondary to TCA OD or hyperkalaemia
- Suspected hyperkalaemia, second line to Calcium Gluconate
- Crush Syndrome.

Contraindications of Sodium Bicarbonate 8.4%?

- Known allergy.

Cautions of Sodium Bicarbonate 8.4%?
-TIA
A
- Ensure adequate ventilation and perfusion to remove the excess CO2 produced
- Tissue necrosis if extravasation occurs
- Ensure IV lines as flushed, avoid mixing with other drugs or solutions.

Side Effects of Sodium Bicarbonate 8.4%?
-RCC
A
- Cerebral oedema
- Congestive cardiac failure and pulmonary oedema.
- Can cause respiratory acidosis if adequate ventilation and circulation is not present.

Dosing Regimes of Sodium Bicarbonate 8.4%?
A
Cardiac Arrest from TCA OD, Hyperkalaemia or Crush
- 100ml IV/IO, single dose
