Pharmacology and presentation of Hartmann’s Sodium Lactate?
1000ml infusion soft pack
Hartmann’s is a sterile solution for intravenous use containing sodium lactate, sodium chloride, potassium chloride and calcium chloride dehydrate. It is considered a balanced crystalloid and has some benefits over normal saline in terms of iatrogenic metabolic acidosis. It is also associated with reduced acute kidney injury post-administration compared to normal saline in certain instances.
Indicators of Hartmann’s Sodium Lactate?
- Hypovolaemic shock
- Burn Resuscitation with Parkland formula.
Contraindications of Hartmann’s Sodium Lactate?
- Known allergy
- Ceftriaxone in same IV line.
Cautions of Hartmann’s Sodium Lactate?
- Failures
- Heart failure
- Renal failure.
Side Effects of Hartmann’s Sodium Lactate?
- Fluid overload in excessive administration.
Dosing Regimes of Hartmann’s Sodium Lactate?
Hypovolaemic Shock
- Adults & Paeds = 20ml/kg in 250ml boluses, continue boluses until per kg amount reached then reassess. PRN to a total of 40mg/kg
Significant Burns (burns <8hrs old and >20% non superficial TBSA)
- Adapted Parkland Volume = (2x non-superficial (1st degree) TBSA % x pt weight in kg) ml for both adult and paeds, infised at a rate that completes administration in 8hrs post burn event, max of 40ml/kg. Consult for Paeds (>18months and <15years)
